Work accident claim form and Requirements
Many people have accidents in their workplace which can result in injury, disability or death at no fault of their own. This can be due to the employer’s or other co-workers’ negligence as a result of a dangerous work environment, fault equipment, inadequate protective clothing and exposure to dangerous chemicals or substances. The employer has the responsibility of providing a safe work environment and take precautions to prevent foreseeable health risks. If certain regulations and requirements are not met by the employer and you have personal injury in the course of doing your work the employer is liable.
Accidents however minor have to be recorded in the work accident book because even minor injuries can become serious later leading to complications. Serious injuries, dangerous accidents and occurrences, death and injuries that require the worker to miss normal duties for more than 3 days have to be reported. The employer is required to report at the Injury Contact Centre for the Health and Safety Executive.
The Report forms have to be filled by the employer and this is mandatory. These include Report of injury and dangerous occurrence report form, Report of flammable gas incidents, Report of dangerous gas fitting, Report of a case of disease, Safety representatives report forms, Safety representatives inspection forms, Report of an accident or dangerous occurrence offshore. These forms can be sent by email or they can be downloaded and filled offline and stored electronically.
It is the sole responsibility of employers to provide their workers with a safe healthy environment and the right equipment, procedures, training and supervision to enable them do their job. Employers have a duty of care towards their employees, customers, suppliers and the general public. They have to ensure your health and safety when you are at the workplace or in the course of duty. Accidents may occur when the employer is negligent. The employer is required to fill the injury occurrence forms stating what happened, the person injured, about the injury and dangerous occurrence.
